All work supported under this Grant shall focus on reducing health disparities for the populations across the state impacted by the high prevalence of cardiovascular disease (CVD), exacerbated by health inequities and disparities, social determinants, such as low incomes, poor health care, and unfair opportunity structures.Β Financial Notes: National Cardiovascular 2304 Strategy 3B ADHS will identify one (1) Subrecipient to create or expand a (CHW) led community-clinical linkage partnership/program in one of the following counties: Apache, Cochise, Coconino, Gila, Graham, Greenlee, Mohave, Navajo, Pima, Pinal, Santa Cruz, Yavapai, or Yuma. Innovative Cardiovascular 2305 Strategies 3A and 3B ADHS will identify one (1) contractor to create or expand a community-clinical linkage partnership/program within Maricopa County. The Subrecipient shall work within a CCL partnerships/programs to identify and train dedicated CHWs (or their equivalents) through a CHW SDOH referral system to create and enhance community clinical links to identify social determinants of health {(SDoH) e.g., housing, transportation, access to care, and community resources} and respond to the individual social services and support needs within each county, providing a continuum of care and services which extend the benefits of clinical interventions and address social services and support needs leading to optimal health outcomes.

The Public Diplomacy Section (PDS) of the U.S. Embassy in Colombo is seeking a local implementing partner to administratively support a four-day Youth Leadership Summit in close consultation with the Embassy, tentatively scheduled for March 2025. The Summit will bring together Youth Forum members representing U.S. Embassy Colombos American Spaces in Kandy, Jaffna, Matara, Batticaloa and Colombos American Center. PROGRAM DESCRIPTION The U.S. Embassy Seoul Public Diplomacy Section (PD) of the U.S. Department of State is pleased to announce that funding is available for a womens leadership program focused on supporting young women across the Republic of Korea (ROK). The ROK is now one of the worlds most influential countries following decades of impressive economic growth, technological innovation, cultural exports, and public sector initiatives. As like-minded democracies, both the ROK and the U.S. have a strong responsibility to make sure economic and social opportunities are open to all citizens, especially women. To support womens economic opportunity, the U.S. Embassy in Seouls Public Diplomacy Section will create a leadership and mentorship cohort of at least twenty South Korean women between the ages of 25-39 who represent diverse sectors of the economy and have shown the potential to grow as leaders in their respective fields. Through this program, participants will learn from American and Korean experts, and broaden their professional network. The U.S. Embassy seeks proposals from ROK-based organizations to run this program which includes recruiting members, planning and implementing events, and expanding its existing network that connects Korean and American women leaders. Strong preference will be given to proposals that include participants from outside of the Seoul metropolitan region and utilize our American Corner network (located in Busan, Gangneung, Gwangju, Jeju and Pyeongtaek) in addition to the American Diplomacy House in Seoul. Outputs/Outcomes This program should focus on developing leadership skills, expanding participants global mindset, and connecting emerging leaders with one another. Details/Logistics U.S. Embassy Seoul and the grantee will be jointly responsible for selecting participants. The U.S. government retains final approval authority for all participants, who will apply via a standardized application form. Participants should be between the ages of 25-39, in their early to mid-careers, and demonstrate their potential to grow as leaders in their professions. Grantee Expectations The following components should be included in the program: Minimum of 6 coaching and/or discussion sessions (either in-person or virtual) Minimum of 2 interactive workshops on leadership Minimum of 2 networking opportunities Coaching Sessions The grantee will be responsible for organizing and implementing a series of in-person, virtual, and/or hybrid engagements with U.S. and/or Korean experts who can empower and provide coaching sessions to this group of selected participants. Engagements should provide opportunities for participants to share their challenges, develop their leadership skills through professional coaching, and provide guidance on their career paths. In-Person Leadership Workshop The leadership program will also host at least 2 in-person workshops at the American Diplomacy House Seoul (or another location of choice), featuring guest experts, opportunities for candid conversations about workplace challenges, and advice for advancing ones career. The Office of the National Coordinator for Health Information Technology (ONC) is accepting applications for a cooperative agreement to measure the use of health information technology (IT) among office-based physicians. The purpose of this cooperative agreement is to measure U.S. office-based physicians' use of interoperable health IT, their experience using health IT, and the impact of health IT on the care they deliver. The cooperative agreement is intended to produce detailed data on interoperability and use of health IT by office-based physicians. These data will result in insights on the implementation and effects of federal health IT policies, allow for identification of disparities in the physicians adopting and using interoperable health IT in terms of the populations they serve or unintended consequences such as increased burden on clinicians resulting from the implementation of health IT, and will inform ONC's ongoing efforts to achieve better health enabled by data through coordination and regulation. Through its partnership with Arizona State Universitys Thunderbird School for Global Management and the Freeport-McMoran Foundation, AWE utilizes a no-cost, online learning platform. Used by AWE since 2019, DreamBuilder teaches participants the fundamentals of starting or growing a small business from a U.S. perspective, offering modules on marketing, pricing, and bookkeeping, and includes a built-in business plan generator. It is available only in English and Spanish, though posts may conduct mentoring and facilitation in local languages. This year, implementing partners may also use a new online curriculum, in whole or in part the Najafi 100 Million Learners Global Initiative - alongside or instead of DreamBuilder. 100 Million Learners Global Entrepreneurship and Innovation Bootcamp may be most suitable to implementing partners who are looking for a more challenging business curriculum, or who wish to provide complementary modules on key topics in entrepreneurship and innovation. 100 Million Learners is currently offered in 20 languages and will add up to 20 more in the future. Background: The U.S. Mission has provided support for USG alumni engagement for five years through the National Alumni Network. Prior programming has included alumni forums, mentoring programs, small grants to regional alumni leaders, and published alumni profiles to build the alumni network. The United States remains a key partner in strengthening Kazakhstan's economy, national security, healthcare, civil society, and other areas of bilateral cooperation. Today, more than ever, Kazakhstan has signaled interest in partnering with the United States in a variety of fields. The U.S. Embassy New Zealand Public Diplomacy Section (PDS) of the U.S. Department of State announces an open competition for organizations to submit applications to carry out a program to develop and implement a Young Pacific Leaders (YPL) Conference sometime between May-July 2025 to provide emerging Pacific leaders with the skills and knowledge they need to become leaders, pending the availability of funding. The Conference should preferably take part in French Polynesia to coincide with the United Nations Ocean Conference being hosted by France in June 2025, but may be hosted in other Pacific locations pending logistics and costs. The State Department has convened a diverse Young Pacific Leaders (YPL) Conference each year since 2013, taking place in American Samoa, Samoa, New Zealand, Hawaii, Fiji, Papua New Guinea, and Guam. Each conference shared the goal of building leadership capacity and creating ties among emerging leaders from across the Pacific region (see below for a list of participating countries). This year, the conference should bring together a group of emerging leaders, ages 25-40, with the goal of providing them with practical leadership skills and a deeper knowledge of regional issues related to the four pillars of the YPL program: civic engagement, environment/resource management, education, and economic and social development. The conference should also help participants grow their networks and enable them to work with their peers on issues that affect the region. The conference should emphasize experiential learning and capacity building in order to equip diverse participants with the tools they need for success. We encourage pre- and post-conference programming that will reinforce the learnings from the conference.
