THIS IS A REQUEST FOR A SECOND YEAR OF SUPPORT FOR A THREE-YEAR INITIATIVE THAT WAS INITIALLY FUNDED THROUGH THE FOUNDATIONS COMMUNITY HEALTH WORKER (CHW) DEMONSTRATION MODEL RFP. WHILE THE RFP LAUNCHED IN 2020, DUE TO DELAYS IN COVID-19, CHARTER HEALTH CENTER, INC., BEGAN THE ASTHMA CONTROL DEMONSTRATION PROJECT IN LATE 2021. AS A FEDERALLY QUALIFIED HEALTH CENTER (FQHC) LOCATED IN HARTFORD, CHARTER OAK SERVES THE HARTFORD AREAS LOWEST-INCOME, MEDICALLY UNDERSERVED AND HOMELESS RESIDENTS. EIGHTY-FOUR PERCENT OF CHARTER OAKS PATIENTS ARE PEOPLE OF COLOR LIVING IN HARTFORDS SOUTH END.

TO ADVANCE THREE OBJECTIVES: 1) RAISE AWARENESS ABOUT HES ADVOCACY PRIORITIES THROUGH TARGETED EVENTS AND TRAININGS THAT ENGAGE HEALTHCARE PROVIDERS, GRASSROOTS ADVOCATES, LEGISLATORS, AND THE GENERAL PUBLIC; 2) ADVANCE PUBLIC DISCOURSE ON ADDRESSING HEALTH EQUITY THROUGH COMMUNITY-LED SOLUTIONS (E.G., COMMUNITY HEALTH WORKERS [CHWS] AND DOULAS); AND 3) BUILD AND EXPAND ISSUE-BASED COALITIONS, SUCH AS THE CHW AND AFFORDABILITY COALITIONS, TO INCREASE MOMENTUM FOR A COORDINATED HEALTH EQUITY ADVOCACY AGENDA. THE ISSUE-BASED COALITIONS WILL ENGAGE BOTH GRASSROOTS AND GRASSTOPS ADVOCACY ORGANIZATIONS TO ADVANCE LEGISLATIVE AND ADMINISTRATIVE POLICY CHANGES.
