LifeCare Health Care Fund

The LifeCare Health Care Fund, a component of the Northwest Minnesota Foundation, was established to provide financial support to qualified nonprofit organizations delivering educational, charitable, and health-related activities, projects, and services that benefit residents and visitors of Roseau County, Minnesota. The fund is administered by the Northwest Minnesota Foundation and overseen by the LifeCare Health Care Fund Advisory Committee. Applications are reviewed by this committee, and applicants may be asked to provide a personal presentation about their proposals. Completed applications must be submitted electronically to Tanya Semingson, Development Coordinator at LifeCare Medical Center. The submission deadline is 4:30 PM on August 28, 2025. Funding is available through several specific funds under the LifeCare Health Care Fund umbrella. The General Endowment supports projects related to health care, wellness, and senior living initiatives that benefit the LifeCare Medical Center service area. The Roseau Eagles Auxiliary Diabetes and Dialysis Fund provides support for diabetic and dialysis needs, including equipment, training, and patient assistance. The Think Pink Breast Cancer Fund supports patient transportation, education, equipment, and other activities related to breast cancer programs in the communities served by LifeCare Medical Center. The Roseau County Cancer Fund finances projects, equipment, and services necessary for cancer care and support of individuals undergoing treatment at LifeCare Medical Center or living in its service area. The grant prioritizes projects that align with the mission and fund purposes, considering the scope of the initiative, the number of participants, the communities served, and the number of organizations involved. Only future projects will be funded; retroactive reimbursements are not eligible. Eligible applicants include nonprofit organizations with current IRS tax-exempt status and public or governmental entities such as townships, cities, counties, or school districts. Private for-profit entities and individuals are not eligible to apply. Applications must address how the proposed project aligns with the mission and specific fund guidelines, clearly describing the scope, expected impact, and the number of people and organizations served. Submissions are made electronically, and applicants should be prepared for the possibility of a follow-up presentation. Following the completion of funded projects, recipients will be required to submit a written evaluation or summary detailing the impact of the grant. The LifeCare Health Care Fund offers financial assistance for a range of health-related needs, including cancer care, diabetes and dialysis services, senior services, hospice programs, medical equipment updates, and resources for families coping with pregnancy or child loss. While the fund operates locally in Roseau County, its multiple funding streams allow it to address targeted health priorities within the community. The Northwest Minnesota Foundation, which houses more than 450 local funds, manages the LifeCare Health Care Fund as part of its mission to strengthen and stabilize communities in northwest Minnesota. The foundation offers other grant opportunities beyond the LifeCare Health Care Fund, and organizations are encouraged to explore these if their project does not meet the eligibility or scope of the current grant.
