Grant Description

NewYork-Presbyterian (NYP) is accepting applications for its 2025 Mini-Grants program, which offers funding to small community-based organizations (CBOs) that support health and wellness initiatives in New York City and Westchester County. NewYork-Presbyterian is a large, integrated academic healthcare system with 10 hospitals and nearly 200 care sites in the Greater New York area. Its mini-grant program was created to support CBOs whose work is critical to community health. Priority will be given to programs serving the neighborhoods of Crown Heights, Corona, Lower East Side/Chinatown, Washington Heights, Inwood, Harlem, Sunset Park, Bedford Stuyvesant, Flatbush, Yonkers, Ossining, Mt. Vernon, Peekskill, and White Plains. These priority areas were identified through NYP’s 2022 Community Health Needs Assessment. The mini-grant provides a three-year funding opportunity. In the first year, successful applicants will receive $5,000. If annual grant requirements are met, funding may be renewed for up to $7,500 in the second year and up to $10,000 in the third year. Eligible projects include new or ongoing health and wellness programs. Proposals should also show a plan for sustainability after the grant ends, demonstrating the project’s potential to continue with other funding or resources. Eligibility for the grant requires that organizations have an annual operating budget of $500,000 or less, possess 501(c)(3) nonprofit status (or work under a fiscal sponsor), and operate independently of NewYork-Presbyterian hospitals or facilities. Funded programs must serve the specified areas in Brooklyn, Queens, Lower Manhattan, Northern Manhattan, or Westchester County and must focus on health and wellness. Projects addressing social determinants of health, such as housing insecurity and employment, are also eligible. Applications will be evaluated based on their alignment with health and wellness priorities, their potential to address community needs, feasibility, clarity of implementation plans, evaluation methods, and realistic budgeting. Applications that support organizational sustainability or serve the identified priority neighborhoods will receive preference. A scoring committee will evaluate proposals, and finalists will undergo follow-up review before final decisions are made. Grantees are required to attend an in-person orientation in early May 2025 following award notifications in late April. They must also participate in two annual check-ins, host at least one site visit during the grant period, and attend at least three NYP-sponsored capacity-building events each year. Reporting requirements include an end-of-year report, an annual survey, and a final budget report. Organizations completing all three years must also complete a post-funding survey within 6 to 12 months after the grant ends.
