Eligible applicants must be registered in the System for Award Management (SAM), have a valid Unique Entity Identifier (UEI), and be registered in DC Health’s Enterprise Grants Management System (EGMS). The application process involves submission of a detailed proposal narrative, logic model, work plan, evaluation plan, risk self-assessment, and various organizational documents including insurance certificates, IRS documentation, and a current board list. All materials must be uploaded via EGMS no later than 3:00 PM on August 12, 2025. Applications will be evaluated based on four weighted criteria: demonstrated need (20 points), implementation strategy (30 points), evaluative measures (25 points), and organizational capacity (25 points).
