Grant Description

The purpose of this Request For Grant Applications (RFGA) is to solicit applications to implement Universal Newborn Nurse Home Visits using the Family Connects International (FCI) model. Expanding evidence-based home visiting is a core strategy to improve the health and developmental outcomes of children in Ohio. These services provide expectant and newly parenting families with structured opportunities for parenting support and individualized care. The model allows nurses to conduct postpartum visits to assess maternal and infant health, respond to clinical questions and concerns, and facilitate referrals to appropriate community resources. FCI model also supports women at risk of complications during the period between childbirth and the postpartum clinical visit. Applicants selected for funding will be eligible to receive start-up support for nurse staffing, training, and supervisory costs. Funding amounts will be determined based on the number of nurses required to meet service delivery needs. Inquiry period closes on October 15, 2025, 8:00am.
