Grant Description

The Adolescent Health Resiliency (AH26) Continuation Solicitation is administered by the Ohio Department of Health (ODH), Bureau of Maternal, Child, and Family Health. This funding opportunity supports first-tier subrecipient agencies to enhance adolescent health programming across Ohio by reinforcing resilience and addressing social determinants of health that impact youth well-being. The initiative is part of a larger Maternal and Child Health Program supported through both state and federal funds, specifically targeting the continuation period from October 1, 2025, through September 30, 2026. The primary objective of the AH26 grant is to sustain and build upon existing efforts to promote adolescent health resilience. Awarded agencies are required to maintain an adolescent health coordinator and implement comprehensive programming aligned with the state’s health improvement goals. Five key deliverables must be addressed: coordinating program leadership, training facilitators in trauma-informed care, updating strategic workplans, engaging advisory committees, and conducting ongoing program evaluation and reporting. Programs are expected to incorporate SMART (Specific, Measurable, Achievable, Results-Oriented, and Time-Based) objectives, informed by tools like the Ohio Health Improvement Zones and Youth Wellbeing dashboard. Applicants must adhere to detailed submission guidelines, including the use of the online Grants Management Information System (GMIS) for application and reporting. Submissions must be electronically received by 4:00 p.m. on May 19, 2025. The application package must include a program narrative, a budget and justification, evidence of progress on reducing health disparities, and various mandatory certifications. Agencies must also comply with federal financial conduct standards and provide documentation such as liability insurance, civil rights surveys, audit reports, and FFATA compliance. Eligibility is limited to first-tier subrecipients who are in good standing with ODH—specifically those who do not owe funds and have not been referred to the Attorney General. Subrecipients must submit monthly and quarterly program and expenditure reports, and failure to comply may jeopardize future funding. Matching funds are not required under this program.
