Eligible applicants include state and local governments, tribal entities, federally qualified health centers, and not-for-profit 501(c)(3) organizations. The application process requires applicants to complete a series of attachments, including budget workbooks, narrative responses, and community assessments. Submissions are to be made electronically through the State of Arizona’s eCivis system, with all required documents uploaded as separate attachments.
