Grant Description

The Minnesota Department of Health (MDH) is offering the Stroke Team-Based Care grant to support innovative approaches that enhance communication, coordination, and collaboration across the continuum of care for individuals at risk for stroke or recovering from stroke. This initiative is part of MDHโ€™s broader efforts under the CDC-funded Paul Coverdell National Acute Stroke Program to improve outcomes for stroke patients in Minnesota. The grant seeks proposals from clinics, healthcare organizations, community-based and social service organizations, Tribal governments, local public health entities, and others experienced in stroke care and prevention. The program aims to fund projects that engage multidisciplinary teams including pharmacists, community health workers, patient navigators, and social workers. These teams will collaborate to improve transitions of care, patient education, follow-up, and community-based interventions. Each awarded project may receive up to $15,000 for a one-year grant term beginning January 2, 2026. There is no cost-sharing or matching requirement. MDH anticipates awarding two grants in total, with the potential for a two-year extension based on performance and funding availability. Applicants are required to submit a detailed work plan and budget, and must clearly align their proposed activities with at least one goal from the Minnesota 2035 Action Plan for Cardiovascular Disease, Stroke, and Diabetes. Grantees will be expected to participate in MDHโ€™s Stroke Learning Collaborative and submit quarterly and annual progress reports. To be eligible, applicants must address at least one priority population (e.g., working-age stroke patients or survivors with unmet needs) and one geographic priority area such as rural counties or communities with high stroke burdens. Applicants working in partnership must submit a single collaborative application with one organization designated as the lead and fiscal agent. Applications must be submitted by 4:30 p.m. CT on October 15, 2025, via the REDCap platform. Required documents include a completed application form, due diligence form, conflict of interest disclosure, and optional letters of support. Awards are expected to be announced in November 2025, with grant activities commencing in January 2026. All expenses must fall within the grant period and be directly tied to project deliverables. MDH will provide technical assistance throughout the project and will monitor progress through regular check-ins and reporting.
