As part of the application, organizations must complete a structured form providing organizational details such as Tax ID, contact information, and confirmation of service to Lyon County residents. Additionally, applicants are required to answer seven narrative questions. These questions cover the problem being addressed, alignment with organizational mission, number of individuals served, available resources, uniqueness and success potential of the project, success measurement, sustainability plan, and other funding efforts. Applicants must also submit a detailed project budget outlining both revenues and expenses.
