Aging in Place Pilot

The New York State Office of Mental Health (OMH) is soliciting proposals for the Aging in Place Pilot initiative, which aims to enhance residential support services for older adults residing in OMH-licensed Community Residence Single Room Occupancy (CR-SRO) programs. This effort seeks to prevent unnecessary institutionalization by providing enhanced supports that enable older and medically complex individuals to remain in their homes. Through this pilot, OMH will award funding to five existing CR-SRO providers across the state, each capable of dedicating 20–25 units for individuals aged 55 and older, or younger individuals with complex medical needs. Each selected pilot program will receive enhanced annual operating funding and up to $2 million in capital funds for necessary renovations or environmental modifications to make the residences safer and more accessible. The initiative targets individuals who, with added supports, can avoid transitions to hospitals, assisted living facilities, or nursing homes. Agencies operating multiple CR-SROs within 30 minutes of each other may apply to deploy enhanced services across those programs. Capital and operating funding approvals are subject to final authorization from the New York State Division of the Budget. Applicants must be not-for-profit organizations with 501(c)(3) status currently operating a CR-SRO with at least 48 units. Proposals must be submitted through the Statewide Financial System (SFS) and must include a budget template, a proposal narrative addressing specific program components, and certifications for sexual harassment prevention and gender-based violence workplace policies. Key elements of the proposal narrative include details on the target population, building modifications, service enhancements, staffing, and diversity and equity strategies. Eligible applicants must be prequalified in the SFS system by the proposal deadline, and they must notify the appropriate Local Government Unit (LGU) prior to applying. Selection criteria will be based on a 100-point scoring rubric, with categories such as population needs, building modifications, program enhancements, diversity and equity initiatives, and financial readiness. The five highest-scoring proposals, one from each OMH region (New York City, Long Island, Hudson River, Central NY, and Western NY), will be selected for funding. The timeline includes a release date of December 9, 2025, with proposals due by February 17, 2026. Awards are anticipated by March 24, 2026, and contracts will begin on July 1, 2026. Each award includes a five-year operating contract and capital funding structured through the Dormitory Authority of the State of New York (DASNY). OMH will provide ongoing oversight, including performance reporting, quality assurance, and learning community participation to ensure the success of each pilot program.
