Grant Description

The Wellness on Wheels - 2025 initiative aims to provide culturally competent and linguistically appropriate preventative health screenings to medically underserved communities and disadvantaged and vulnerable communities of color in Illinois. This program focuses on reducing health disparities and inequities through services such as HIV, Hepatitis C, and other sexually transmitted infection screenings. Services must include partnerships to provide linkage to care and other wrap-around services as needed. Eligible applicants must have the ability to adjust outreach plans based on client input, disease outbreaks, and community events. Awards are funded through the State of Illinois general revenue and total $995,000. Applications must be submitted by August 8, 2024.
