The Intermountain Community Care Foundation (ICCF) is a private foundation dedicated to advancing the mission of Intermountain Health by improving community health outcomes. The Foundation focuses on identified health priorities, enhancing access to healthcare services, and promoting healthy behaviors among low-income, uninsured, or medically underrepresented populations in Utah and Southeast Idaho, specifically Cassia County and Minidoka County. By supporting existing programs and services that address these populations’ needs, ICCF aims to help people live the healthiest lives possible. Funding priorities are informed by Intermountain’s Community Health Needs Assessment (CHNA), which identifies significant health challenges in the region. Funding is available for projects that fall within four main categories: Agency Health Priority, Social Drivers of Health, Child & Family Mental Well-Being, and Education Innovation. Under Agency Health Priority, projects may address mental well-being, prevent chronic illnesses, and improve access to healthcare. The Social Drivers of Health category supports agencies addressing broader factors influencing health outcomes. Child & Family Mental Well-Being grants are for programs promoting mental wellness among children and families, while Education Innovation funds efforts to improve educational outcomes in Utah. Across all categories, ICCF seeks innovative, measurable, cost-effective projects that create lasting change and improve access for underserved populations. Applications must be submitted through ICCF’s electronic portal powered by Versaic. The process requires creating an account and submitting several mandatory documents: a full and current audited financial statement (within two years), proof of 501(c)(3) status, a completed W-9, an itemized budget for both the organization and the proposed program, and current lists of the board of directors and key staff. Applications missing any of these items will be disqualified. Large equipment, construction, political activities, debt retirement, travel expenses, and other specified uses are not eligible for funding. Additionally, the grant request may not exceed 20 percent of the applicant organization’s annual operating budget, and awards range from $5,000 to $50,000. The application windows occur twice yearly: January 1–February 1 for consideration at the May board meeting, and July 1–August 1 for the November board meeting. Applications must be fully completed within these windows to be reviewed. Organizations may only hold one active grant at a time and must wait 18 months after being funded to reapply, or until the final outcomes report is reviewed if funded for multiple years. Outcome measures reports are due approximately one year after grant payment and must detail measurable human impacts and program results. Grantees may be subject to site visits and reviews during the funding period. Eligible applicants include nonprofit organizations with 501(c)(3) status serving Utah and the specified Idaho counties, whose programs align with ICCF’s funding categories. Projects must be accessible to the public on a nondiscriminatory basis. Religious organizations are eligible only if services are offered without requiring worship or adherence to a faith tradition. The board evaluates proposals based on alignment with funding priorities, financial health, and potential impact, and has sole discretion over award decisions. Applicants can request a total grant award between $5,000 and $50,000 for the entire project period. A funding request may not exceed 20 percent of an organization’s annual operating budget, e.g., to be eligible for a $50,000 grant, your organization’s annual operating budget must be at least $250,000. In general, small equipment and materials are eligible at no more than a total $5,000 cost, but large equipment, construction, and capital costs are not eligible. Consideration will be given if computer equipment is necessary and vital to the success of the project.
