The Level Up LB: Extreme Small Business Makeover and Grant Program is an initiative designed to propel small businesses towards both success and long-term sustainability. Individual artists, artist collaborators, not-for-profit organizations, and the cities of Alachua, Archer, Hawthorne, High Springs, LaCrosse, Micanopy, Newberry, and Waldo are eligible to apply if the following criteria are met: If an artist, the applicant must: • Be a resident of Alachua County, Florida • Be a US Citizen • Be 18 years or older 3 If an organization: • Recognized as a tax-exempt 501(c)3 • Has a Unique Entity Identifier (UEI): Organizations MUST have a valid UEI to receive federal funding (previously a DUNS number).
