Grant Description

Con Alma Health Foundation (CAHF) offers multi-year grants to support nonprofit organizations dedicated to improving health and promoting health equity for communities across New Mexico. The foundation’s goal is to ensure that all individuals in the state, particularly those in historically marginalized and medically underserved communities, have fair and equitable opportunities to achieve good health. This funding initiative prioritizes systemic change rather than individual-level services and emphasizes a broad approach to health, encompassing education, empowerment, policy change, and workforce development. In the 2025 funding cycle, applicants can request $30,000, $40,000, or $50,000 annually for up to three years, amounting to a maximum of $150,000 per organization. CAHF has allocated up to $300,000 in total for this round of multi-year grants. The grant implementation period spans from January 1, 2026, through December 31, 2026. The foundation supports six priority areas: access to healthcare, culturally appropriate services, evaluation and research, health promotion, health policy advocacy, and workforce development that reflects the cultural and linguistic diversity of New Mexico communities. Eligible applicants must be nonprofit organizations with 501(c)(3) status or operate under a fiscal sponsor that holds such designation. They must demonstrate at least three years of successful programming. Work proposed under the grant must directly benefit people living in New Mexico, with a special emphasis on communities facing systemic barriers, including rural, tribal, immigrant, and low-income populations. The grant application process begins with a Letter of Intent (LOI), open from April 1 to April 15, 2025. Selected applicants will be invited by May 28 to submit a full application between June 2 and June 30, 2025. Award decisions will be communicated in November 2025, and grants will be disbursed in December 2025. The LOI and full applications will be evaluated based on alignment with CAHF’s mission and goals for systemic health improvement. Full applications require several attachments, including a three-year proposal budget, the organization's current operating budget, financial audit summaries from the past three years, and tribal support letters for projects involving tribal communities. Applications using a fiscal sponsor must submit documentation of the sponsor’s nonprofit status and complete an organization profile in CAHF’s grant portal. Grant funds may be used for staffing, professional services, operational and communication costs, training and travel, and both direct and indirect expenses. Equipment purchases are limited to $5,000 per item. Prohibited uses include political activities, scholarships, religious purposes, and capital campaigns. Applicants must register on CAHF’s SmartSimple portal and maintain accurate organizational profiles, including demographic data for staff and leadership.
