Grant Description

The New York State Department of Health, through its Division of Chronic Disease Prevention, announces funding under the Tobacco Control Program (TCP) for Component A: Health Systems for a Tobacco-Free New York. The initiative is designed to engage and support health care organizations in implementing system-level and policy-level changes that improve the delivery of evidence-based care for Tobacco Use Disorders, including nicotine dependence. The program is part of the Department’s ongoing effort to reduce the illness, disability, and death associated with tobacco use and secondhand smoke exposure across the state. The Bureau of Tobacco Control (BTC) administers this component as part of the state’s broader comprehensive Tobacco Control Program. The program integrates tobacco-free community initiatives, cessation support, health systems improvement, and paid and earned media efforts. Applicants are expected to demonstrate capacity to influence health system operations, clinical workflows, and organizational policies to ensure guideline-concordant care for tobacco dependence. Eligible applicants include governmental entities and not-for-profit organizations with the ability to engage healthcare systems and influence clinical practice. Funding will support the development and implementation of systems and environmental changes, not direct patient services. Projects should align with the state’s goal to alter social norms surrounding tobacco and nicotine use—making such behaviors less acceptable, desirable, and accessible. Applicants are encouraged to partner with relevant state and national stakeholders to strengthen outcomes and ensure sustainability. The anticipated total funding available under this opportunity is approximately $14,796,875, distributed across multiple awards with a 60-month performance period.
