CareOregon Community Grants 2025
This funding opportunity supports community-based organizations in Oregon that address food, housing, and healthcare access for Medicaid and low-income individuals.
CareOregon, a nonprofit organization dedicated to supporting the health needs of Medicaid and low-income populations across Oregon, has announced its 2025 Community Giving initiative. With over two decades of investment in local communities, CareOregon has directed more than $85 million since 2020 into grants, sponsorships, and other support for community-based organizations. The program reflects CareOregon’s broader mission to partner with others to achieve quality and equity in health for individuals and communities. The purpose of the 2025 Community Giving program is to address key social determinants of health that impact members in CareOregon’s service areas. In 2025, funding will prioritize three main focus areas: Food & Nutrition Education, Housing Support, and Access to Care & Navigation Services. Examples include community gardens and teaching kitchens under the food and nutrition category; eviction prevention and resident services under housing; and culturally specific navigation and health literacy programs under access to care. CareOregon emphasizes collaboration and prioritizes applications from culturally and linguistically specific organizations, as well as those led or staffed by Traditional Health Workers (THWs), including Community Health Workers and Peer Support Specialists. Applicants must serve populations within Clackamas, Clatsop, Columbia, Jackson, Multnomah, Tillamook, or Washington counties in Oregon. Programming must support Medicaid or low-income individuals within these regions. Programs that exclude individuals based on age, disability, gender identity, marital status, national origin, race, religion, color, sex, or sexual orientation are not eligible for support. Grant amounts generally average $25,000, while event sponsorships typically average under $5,000. Organizations seeking funding above the usual amounts, or those that do not already have an established relationship with CareOregon, are encouraged to contact the grant program team prior to submission. Sponsorships must support community events that are free to the public and located within CareOregon’s designated service area. Applications for 2025 community grants are accepted from June 2 through July 31, 2025. Notifications of funding decisions will be sent by September 8, 2025. For sponsorships, applications must be submitted before September 1, 2025 or at least 90 days prior to the event date. CareOregon does not fund events outside its geographic focus or support fundraisers from non-partner organizations.
