The CCME Foundation, part of Constellation Quality Health, is a charitable entity established in 2018 to honor the organization’s four-decade legacy of improving healthcare access and outcomes. The Foundation’s mission is to support nonprofit organizations through grants that enhance access to health care and related services across communities in North Carolina and South Carolina. Its grantmaking efforts focus particularly on underserved and uninsured populations and aim to foster measurable improvements in healthcare quality and equity.
Since its inception, the CCME Foundation has awarded over $1.5 million in grants to community-based programs. These programs have addressed various health improvement priorities, including healthy lifestyles, culturally appropriate care, health education, consumer-centered decision-making, and patient safety. Through its Promising Practices Framework, the Foundation emphasizes the support of innovative and effective grassroots solutions, especially in rural and sparsely suburban areas, though urban initiatives are also eligible for funding.
The Foundation awards grants ranging from $5,000 to $50,000 per organization. The funding is prioritized for projects that demonstrate measurable outcomes and align with focus areas such as post-acute care, behavioral health, chronic disease management, and workforce development. While innovation and partnerships are encouraged, all projects must maintain a strong focus on improving healthcare access and outcomes.
Eligible applicants are nonprofit organizations operating within North Carolina and South Carolina. The Foundation especially welcomes applications from community-based organizations in the early stages of development. The annual grant application window opens on April 1 and closes on June 15. All applications must be submitted within this period.
