Research to Spread and Scale the Impact of Evidence-Based Falls Prevention Programs and Interventions
This funding opportunity is designed to support non-profit organizations in expanding and implementing proven falls prevention programs for older adults, focusing on effective strategies for broader adoption within community and clinical settings.
The Administration for Community Living (ACL), under the U.S. Department of Health and Human Services (HHS), is offering funding through its Center for Innovation and Partnership for research aimed at expanding and evaluating evidence-based multifactorial falls prevention programs. The funding opportunity is titled “Research to Spread and Scale the Impact of Evidence-Based Falls Prevention Programs and Interventions” and is identified under funding opportunity number HHS-2025-ACL-CIP-AAFP-0051. This initiative supports implementation science focused on scaling proven falls prevention interventions through partnerships with the Aging Network and community-based organizations. The grant opportunity seeks to bridge the gap between research and real-world application by focusing not on the development of new interventions, but on implementing and evaluating existing evidence-based programs. This includes structured data collection, fostering cross-sector collaboration, providing technical assistance, and conducting predictive and statistical analysis. The goal is to understand and measure the population-level impact of scaling falls prevention efforts among older adults (defined as age 60 and above, with tribal organizations defining their own threshold for “older adults”) and to build sustainability for community-based delivery beyond the funding period. Eligible applicants include a wide range of domestic public or private non-profit entities, such as state and local governments, tribal governments and organizations, educational institutions, community-based organizations, public housing authorities, and nonprofits with or without 501(c)(3) status. Foreign entities are not eligible. Importantly, applicants must demonstrate significant expertise in implementation research and existing partnerships or experience working with the Aging Network or community care hubs. Submissions must include letters of commitment from relevant Aging Network partners such as Area Agencies on Aging, State Units on Aging, or tribal organizations. Applications missing these letters will not be considered. Applicants may request up to $4.65 million for a 48-month (4-year) forward-funded project. Each project must include research that uses implementation science methodologies, examines effectiveness in real-world settings, and provides detailed plans for data sharing, sustainability, and dissemination. Applicants must also budget for personnel, programming, technical assistance, and participation in aging or falls-related conferences annually. The application process includes submitting a letter of intent by July 31, 2025, although this is not mandatory. The final application must be submitted through Grants.gov by August 20, 2025, at 11:59 p.m. Eastern Time. The anticipated award start date is September 1, 2025. Review criteria include the significance of the research, the methodological approach, organizational capacity, a detailed work plan, budget justification, and evidence of strong collaborative partnerships.
