Grant Description

The Intermountain Community Care Foundation is a private grantmaking organization dedicated to advancing the mission of Intermountain Healthcare to help people live their healthiest lives possible. The Foundation focuses on supporting programs that improve access to healthcare services, increase healthy behaviors, and address priority community health needs for low-income, uninsured, and medically underrepresented populations in Utah and Southeast Idaho, specifically Cassia and Minidoka Counties. Funding priorities are based on the Community Health Needs Assessment (CHNA), which identifies significant health needs within the service areas. Grant funding is available for programs that align with specific priority areas: Agency Health Priority, which includes mental well-being, prevention of chronic illnesses, and improved healthcare access; Social Drivers of Health, which addresses underlying social factors affecting health; Child and Family Mental Well-Being, which targets mental health for children and families through prevention, intervention, and trauma-informed care; and Education Innovation, which supports enhanced educational outcomes in Utah. Applicants are encouraged to propose innovative, measurable, and cost-effective projects that create lasting positive change and increase access for target populations. The Foundation accepts applications twice annually, with submission periods from January 1 to February 1 for consideration in May, and from July 1 to August 1 for consideration in November. Applications must be submitted electronically via Versaic, with all required materials uploaded, including a full audited financial statement from within the last two years, proof of 501(c)(3) or equivalent status, a completed W-9, an organizational budget, a program budget, and current lists of board members and key staff. Programs must fit within the Foundation’s funding categories to be eligible, and applicants should confirm eligibility with staff if uncertain. Funding requests can range from $10,000 to $100,000 per year for up to three years (maximum $300,000) for projects lasting 18 to 36 months, beginning upon receipt of funds. Requests may not exceed 20% of an organization’s annual operating budget, and certain costs—such as large equipment, construction, capital projects, and political activities—are ineligible. Small equipment and materials may be funded up to $5,000, and computer equipment may be considered if essential to project success. Grants are awarded in full after the signed agreement is returned, and grantees must submit an outcomes report approximately one year after payment. Eligibility is limited to organizations that meet the mission alignment and documentation requirements. Recipients may not apply again for 18 months following funding, or until after the final report for multi-year awards is reviewed. Organizations may only hold one active grant at a time. The Foundation Board evaluates proposals based on alignment with criteria, available funding, and potential impact. Not all qualified proposals will receive funding, and amounts awarded may differ from the request. Applications must demonstrate a clear fit with funding categories, have measurable objectives, and show potential for significant and lasting change.
