Live Well Grants Program

The Live Well Grants Program is administered by Mecklenburg County Public Health, Community Grants & Partnership, to support grassroots, community, and faith-based organizations in implementing chronic disease prevention initiatives. Only nonprofit 501(c) organizations or faith-based organizations with a valid tax ID and active vendor status with Mecklenburg County may apply. Organizations must also be volunteer-driven or have no more than ten full-time staff. Proof of general liability, auto, and workersโ compensation insurance is required, and monthly reporting of activities and outcomes is mandatory. Grant-funded activities must begin within 30 days of the award and must target eligible chronic disease prevention categories. Supported program areas include tobacco prevention and cessation, healthy eating and nutrition, Type 2 diabetes prevention and management, heart disease and stroke prevention, cancer prevention, obesity prevention and active living, and preventive health screenings. Programs must be conducted within the county and directly benefit local residents. Activities such as full-time staffing, regional or national initiatives, research, travel, and retroactive expenses are not eligible for funding. The 2025โ2026 grant cycle offers a total of $125,000 in funding. Each selected organization may receive up to $15,000 in reimbursable grant support.
