Required materials include the online application form, the most recent audited and unaudited financial statements, and an IRS determination letter verifying 501(c)(3) status. Eligibility criteria are clearly defined. Only 501(c)(3) nonprofit organizations serving targeted populations are eligible to apply. Organizations may not submit applications for two consecutive grant cycles and must take a one-year hiatus after receiving grants in two consecutive years.
