Community-Clinical Linkage Partnership/Program

The Community Health Worker (CHW) Community-Clinical Linkage Partnership/Program grant (RFGA2025-012) is offered by the Arizona Department of Health Services (ADHS) through its Bureau of Chronic Disease and Health Promotion (BCDHP). This grant program aims to support the development of partnerships that connect community health workers (CHWs) with clinical settings to address chronic diseases such as heart disease, stroke, lung disease, cancer, diabetes, and asthma. These diseases are among the most common, costly, and preventable health problems in Arizona. The initiative is designed to build individual, organizational, and community capacities to reduce the burden of chronic diseases by fostering collaboration between health and social service providers, community organizations, and clinical care providers. One award of $100,000 will be made to a public or nonprofit organization that demonstrates capacity and experience in linking community health programs with clinical care. Eligible applicants include public or nonprofit health and social service providers, 638 tribal facilities, community-based organizations, academic institutions, schools, city or county governments, and other nonprofit entities based in Arizona. Applications must include plans for sustainable partnerships, measurable outcomes, and interventions aligned with the goals of reducing the impact of chronic diseases in underserved communities. Applications are due by March 6, 2025, and the awarded project must be implemented in Arizona.
