Grant Description

The Critical Time Transition Program (CTTP) for Children, Youth, and Families is a statewide initiative funded by the New York State Office of Mental Health (OMH) to address the needs of youth with complex mental health challenges transitioning from higher levels of care. The program targets youth aged 11–17 (and up to 19 in some cases) who face extended delays in discharge from psychiatric or emergency departments due to the lack of appropriate community services. CTTP includes two integrated components: Critical Time Intervention (CTI) teams and Transitional Residential Settings (TRS), providing temporary housing, mental health care, and intensive family engagement to support successful transitions back to community living. OMH plans to establish up to four CTTP teams in various regions across New York State. These programs use evidence-based practices and a trauma-informed care model to address critical barriers to community integration and stabilization for youth and their caregivers. Programs will offer comprehensive assessments, wraparound services, and a multidisciplinary care model. Funding includes both operating costs and capital funding for developing transitional residential settings.
