Grant Description

This funding opportunity, CDC-RFA-DP-25-0024, is designed to support public health efforts in the U.S. territories and freely associated states to address chronic diseases through evidence-based and culturally tailored strategies. Chronic diseases are a significant health burden, and this grant aims to reduce disability and death rates by targeting modifiable risk factors such as tobacco use, diabetes, and oral health disparities. The initiative is divided into two components. The mandatory Core Component focuses on integrated strategies to reduce tobacco use, manage diabetes, and improve related health outcomes. The optional Oral Health Component addresses oral health disparities through interventions such as preventive dental services and infection control in dental settings. Applicants for the Oral Health Component must also apply for the Core Component. The funding, distributed over a five-year period, totals $17,450,000, with individual annual awards averaging $475,000 for the Core Component and $200,000 for the Oral Health Component. Eligible applicants include a wide range of organizations such as state and local governments, nonprofits, institutions of higher education, tribal organizations, and small businesses. Applicants must demonstrate their capacity to access and submit relevant health data and provide proof of location within the eligible territories or freely associated states. Collaboration with local health departments and community stakeholders is emphasized to ensure the success and sustainability of the initiatives. Applications must include a project narrative, work plans, budget justifications, and supporting documentation such as an organizational chart, staffing plans, and letters of support. A detailed evaluation and performance measurement plan is also required. The deadline for submission is January 15, 2025, with the project expected to start on March 31, 2025. The review process assesses applicants on their background and approach, organizational capacity, and plans for evaluation and performance measurement. If the applicant is other than a Department/Ministry of Health in the specified jurisdiction, the applicant must submit a memorandum of agreement/memorandum of understanding with the Department/Ministry of Health in the jurisdiction for which it is applying. CDC will need certain surveillance and health system data from applicants in order to ensure performance measures are being met for the benefit of achieving public heath results and hence, applicants must demonstrate they have the authority to submit the data to CDC and will have access to the data. The MOU/MOA should specify that the Department/Ministry of Health will provide the applicant access to surveillance and health system data, has granted the applicant the authority to submit the data, and will collaborate with the applicant on this NOFO to ensure complete data are submitted to CDC according to established guidelines and processes. The MOU/MOA must be provided in an attachment labeled MOU/MOA; when submitting via www.grants.gov. Foreign Institutions, Non-domestic (non-U.S.) components of U.S. Organizations are not eligible to apply. Non-domestic (non-U.S.) Entities (Foreign Institutions) are not eligible to apply. CDC will consider any application requesting an award higher than the ceiling amount of $1,400,000 to be non-responsive. Applicants must provide proof of ability to serve populations in one of the jurisdictions listed below: Commonwealth of the Northern Mariana Islands (CNMI), American Samoa, Guam, Federated States of Micronesia (FSM), Republic of the Marshall Islands (RMI), Republic of Palau, U.S. Virgin Islands (USVI), Puerto Rico, Evidence of ability may include documentation that the applicant is a territorial government or a bona fide agent in the CNMI, American Samoa, Guam, the FSM, the RMI, the Republic of Palau, USVI, or Puerto Rico. Documentation must be provided in attachment labeled Evidence of Ability; when submitting via www.grants.gov. A bona fide agent is an agency/organization identified by the state as eligible to submit an application under the state eligibility in lieu of a state application. If applying as a bona fide agent of a state or local government, a legal, binding agreement from the state or local government as documentation of the status is required. Attach with Other Attachment Forms; when submitting via www.grants.gov.Applications that do not comply with these requirements will be determined non-responsive and will not receive further review.
