Grant Description

Con Alma Health Foundation (CAHF) is a private grantmaking organization dedicated to advancing health equity and improving health outcomes for the people of New Mexico. With a commitment to serving the state’s diverse and underserved populations, the foundation funds nonprofit organizations that align with its mission of community-informed, system-level change in health care access and outcomes. The foundation's strategy emphasizes long-term policy solutions and values community voices as central to programmatic design and implementation. The 2025 CAHF Grant Cycle supports one-year projects with award amounts of $15,000, $20,000, $25,000, or $30,000. The total anticipated funding for this cycle is up to $800,000. Projects must benefit residents of New Mexico and align with one or more of six priority areas: increasing access to health and healthcare for the uninsured and underserved; promoting culturally appropriate services for rural, tribal, immigrant, and racially diverse communities; supporting evaluation and research for policy and advocacy; emphasizing health promotion through education and prevention; increasing consumer participation in health policy; and developing a culturally and linguistically representative health workforce. Applicants must be tax-exempt nonprofits under Section 501(c)(3) or use a fiscal sponsor that meets these requirements. Grants are not provided to individuals, religious organizations, political activities, or for capital expenditures, scholarships, or construction-related projects. The application must be submitted through Con Alma’s grant portal and include a completed proposal budget, a current organizational operating budget, and, if applicable, a tribal support letter for work conducted within tribal jurisdictions. Extensions for missing documents may be granted if a written request is submitted at the time of application. Applications open on May 2, 2025, and close on May 30, 2025, at 11:59 PM. Notifications will be sent in November 2025, with funds disbursed in December 2025. The grant implementation period runs from January 1 to December 31, 2026. Proposals will be reviewed through a three-stage process: an internal review for eligibility and completeness, a technical review by community evaluators, and a final assessment by the Grant Making Committee focused on mission alignment and impact. Con Alma uses an online system that requires applicants to create an organizational profile and answer a brief quiz that determines eligibility and guides users to the appropriate application form.
