The primary objective of the HFLI program is to develop and deliver a comprehensive family health intervention program using the "Healthy Weight and Your Child" curriculum, an evidence-based model approved by the CDC. The program targets black and/or Latino families in Peoria, reflecting the Department’s emphasis on health equity and targeted service delivery. The first year of the grant, covering July 1, 2025 through June 29, 2026, is intended to support planning and early implementation activities, including staff training, partnership development, and program acquisition. By the end of the first year, grantees must submit an implementation plan outlining milestones and a timeline for the subsequent two years of programming. To be eligible, applicants must be tax-exempt organizations operating in Peoria, Illinois. Additionally, applicants must be registered and prequalified through the Illinois GATA portal and meet multiple compliance checks including registration with SAM.gov, a valid UEI number, and good standing with the Illinois Secretary of State if applicable. The selected organization must demonstrate experience working with children and families, existing community trust, and the operational capacity to act as an implementation partner for a public health pilot. A strong application will detail prior relevant work, defined partnerships, and the ability to deliver CDC-compliant programming in a culturally appropriate manner. The total funding available is $20,000, with no cost-sharing or match requirement. One award will be issued to cover activities associated with program provision, host site coordination, and referral partner engagement. The funds may be used for personal services, supplies, travel, and certain contractual and indirect costs, with strict limitations on unallowable expenses like food, equipment purchases, or facility upgrades. A minimum of 10% of the budget must be allocated to address social determinants of health that impact program participation. Grantees will be required to submit quarterly reports, participate in monthly calls with IDPH, and seek prior approval for any promotional materials. Applications must be submitted via IDPH’s EGrAMS platform between June 30, 2025 and August 15, 2025 by 5:00 PM. The application must include a detailed project plan, letters of commitment from partners, and a clear sustainability strategy. Organizations with sufficient progress during the first year may be eligible for up to two years of non-competitive renewal funding.
