Local Models Program 2024

The 2024 Community Care Corps grant program seeks to fund innovative volunteer models that provide nonmedical assistance to support older adults, people with disabilities aged 18 and older, and their family caregivers. Managed by the Oasis Institute with federal funding from the Administration for Community Living (ACL), this initiative aims to increase community-based volunteer programs that help maintain independence and reduce unmet needs. The program will evaluate and share effective models and resources through a Learning Library to support similar initiatives nationwide. Eligible applicants include a diverse array of entities, such as nonprofits, faith-based organizations, community-based organizations, state and local governments, institutions of higher education, and tribal organizations. Applicants are expected to develop, expand, or refine volunteer models addressing unmet community needs, particularly through initiatives focused on supporting family caregivers, leveraging technology, and using volunteers to assist in non-emergency medical visits. These models should promote independence and increase access to nonmedical support, without including direct medical care. Grants range from $30,000 to $200,000 for an 18-month project period. Awardees must contribute a 25% match of the total project budget through cash or in-kind contributions. Funding operates as a reimbursement grant, with eligible costs covered quarterly upon submission of financial reports. Key dates include: Application period: October 28, 2024, to December 11, 2024. Applicants must demonstrate community involvement, define their service population, and include sustainability and partnership plans. The application process involves an online submission portal, an optional informational webinar, and rigorous reporting and evaluation commitments. Final selections will be based on criteria such as project innovation, target population relevance, and geographic diversity.
