Grant Description

The New York State Office of Mental Health (OMH) is seeking proposals for the implementation of five Adult Critical Time Intervention (CTI) Teams as part of Round Three of the CTI program. These teams aim to support individuals with complex mental health conditions during critical transitions, such as discharge from psychiatric hospitals or emergency departments, focusing on building community integration, fostering self-advocacy, and ensuring long-term support. The program emphasizes evidence-based interventions to strengthen linkages between individuals and professional or non-professional support networks during transitional periods. CTI Teams are designed to address key social and clinical needs through assertive outreach, individualized planning, and close collaboration with hospitals and community providers. Teams will operate under structured, phase-based models tailored to community and regional needs, providing personalized support and connections to services.
