General Grant Request

The Delta Dental Foundation (DDF) is a nonprofit, charitable organization founded in 1980. It serves as the philanthropic arm of Delta Dental of Michigan, Ohio, Indiana, and North Carolina. Projects intended for political activities, campaigns, loans, or commercial/proprietary purposes are also ineligible. Applicants must submit a completed grant application form to be considered. If awarded funding, recipients are required to submit a grant reporting form either within one year of receiving the funds or before requesting additional funds, whichever comes first. The grant reporting form ensures accountability and ongoing alignment with DDF's mission and standards.
