The Community Health Foundation offers Responsive Grants to support health and wellness programs that serve residents in Clark or Champaign County, Ohio. A private foundation, Community Health Foundation provides funding to qualified 501(c)(3) or other public-serving organizations operating within the specified geographic area. Their grantmaking aligns with county-level health priorities and supports a broad array of initiatives that address public health needs.
Responsive Grant applications are divided into two categories based on funding amounts. For requests of $5,000 or less, a simplified application is provided. For those requesting more than $5,000, a separate, more detailed application must be completed. All grant applications must be submitted online through the Smarter Select portal. New applicants are required to attend a Grant Information Session held in November, while returning applicants may attend voluntarily. These sessions help clarify the application process and improve proposal quality.
Grant funds are intended to support community-based initiatives that enhance health outcomes, consistent with the goals outlined in the Clark and Champaign County Community Health Improvement Plans. Funding priorities include chronic disease prevention and management, maternal, infant, and sexual health, mental health, and housing and neighborhood stability. Applicants are encouraged to align their programs with these areas for the best chance of success.
The application window for the 2026 Responsive Grant cycle is open, with a submission deadline of February 13, 2026, at 5:00pm Eastern Time. Determination letters are expected to be mailed in early May. There is no stated requirement for cost-sharing or matching funds in the current grant cycle. Budget templates are provided, but applicants may submit their own budget format if preferred.
In addition to Responsive Grants, Community Health Foundation offers Mini Grants of up to $1,000 through a separate, streamlined process. These mini grants are accepted on a rolling basis and must also support health and wellness efforts within Clark or Champaign County. Applicants for Mini Grants are instructed to notify Faith Bosland before submission.
