Grant Description

The *Community Health Aide Program: Tribal Assessment & Planning* grant (HHS-2025-IHS-TAP-0001), funded by the Indian Health Service (IHS), aims to support Tribes and Tribal Organizations (T/TOs) in assessing the feasibility of implementing a Community Health Aide Program (CHAP) within their communities. This grant provides an opportunity for T/TOs to evaluate the need and potential impact of CHAP, which is tailored to address healthcare needs in rural and underserved Tribal areas. The program seeks to facilitate flexible, community-specific planning to address local health challenges and ensure access to quality healthcare. This forecasted grant opportunity offers a total funding pool of $1,500,000, with individual awards ranging between $250,000 and $500,000. The program expects to issue four awards to eligible applicants. Funding is discretionary and intended solely for federally recognized Indian Tribes and Tribal organizations, as defined under U.S. Code 25 U.S.C. 1603(14) and 25 U.S.C. 1603(26). Applications must include letters of support or Tribal Resolutions from the relevant communities or Tribes that will benefit from the program. Additionally, entities cannot hold concurrent awards under this program, so once awarded, applicants are ineligible to apply for similar funding within the same project period. The primary objective of this funding opportunity is to enable T/TOs to conduct a thorough assessment of the specific health needs of their communities and to develop a strategic plan for CHAP implementation. The program encourages a customized approach, allowing each applicant to identify unique regional and cultural factors that may influence program design. The assessment and planning activities funded through this grant are expected to cover the identification of local healthcare needs, resource requirements, and implementation logistics, which will ultimately support the feasibility of establishing a CHAP model tailored to each community. Currently, the grant opportunity remains in the forecast stage, with the official application package expected to be released on May 1, 2025. The application due date and further submission details will be available once the funding opportunity is fully posted on Grants.gov. Interested applicants should closely monitor this listing for updates on submission requirements, and they are encouraged to prepare by organizing letters of support and reviewing local healthcare needs to align with CHAP goals.
