Street Medicine Interventions for People with HIV who are Unsheltered Capacity Builder Provider (HRSA-25-055) and Street Medicine Interventions for People with HIV who are Unsheltered Evaluation Provider (HRSA-25-057)

The Health Resources and Services Administration (HRSA) HIV/AIDS Bureau (HAB) is seeking applications for fiscal year 2025 under two related funding opportunities: Street Medicine Interventions for People with HIV who are Unsheltered – Capacity Builder Provider (HRSA-25-055) and Street Medicine Interventions for People with HIV who are Unsheltered – Evaluation Provider (HRSA-25-057). These grants aim to enhance healthcare access and treatment outcomes for unsheltered individuals living with HIV by implementing and evaluating street medicine interventions. The initiative is part of the Ryan White HIV/AIDS Program (RWHAP) Special Projects of National Significance (SPNS) and emphasizes an implementation science approach to documenting, evaluating, and disseminating effective healthcare delivery models for unsheltered individuals. The Capacity Builder Provider (HRSA-25-055) will be responsible for coordinating activities across the initiative, providing technical assistance to demonstration sites, and ensuring that interventions are equitable, client-centered, and scalable. Key responsibilities include strategic planning, learning sessions, annual multi-site meetings, and the development of a communications and dissemination plan to support program replication. The Evaluation Provider (HRSA-25-057) will lead a rigorous multisite evaluation to assess implementation outcomes, service effectiveness, and cost implications, while also supporting data collection, analysis, and training at demonstration sites. The initiative is focused on developing and expanding street medicine, which involves delivering healthcare directly in public spaces where unsheltered individuals reside. The targeted populations include individuals living with HIV who are not engaged in consistent care, particularly those experiencing homelessness. The interventions will address HIV treatment engagement, viral suppression, and the broader health challenges faced by this population, including substance use disorders and mental health concerns. Eligibility is restricted to specific domestic entities, including public and private institutions of higher education, nonprofit organizations (with or without 501(c)(3) status), state and local governments, independent school districts, and Native American tribal governments or organizations. Individuals are not eligible to apply. The program offers one award under each funding opportunity, with HRSA-25-055 providing up to $1,250,000 per year and HRSA-25-057 offering up to $1,000,000 per year, for a total of four years of funding from August 1, 2025, to July 31, 2029. Applications must be submitted through Grants.gov by March 11, 2025, at 11:59 p.m. Eastern Time. An informational webinar will be announced at a later date, and applicants are encouraged to subscribe to updates. The expected award announcement date is July 2, 2025, with project implementation beginning on August 1, 2025. The initiative requires collaboration between the Capacity Building Provider, Evaluation Provider, and 10 Demonstration Sites (funded under HRSA-25-056) to implement and evaluate street medicine interventions effectively. The application process requires a comprehensive project narrative, including work plans, budget justifications, and key personnel details. Evaluation criteria include demonstrated need (10 points), response to program requirements (40 points), evaluation measures (10 points), anticipated impact (10 points), organizational resources and capabilities (25 points), and budget justification (5 points). The review panel will assess each application based on these criteria to ensure alignment with program goals and sustainability planning.
