The IMPLICIT: Innovations for Maternal and Perinatal Care and Improvement Grant is administered by the Minnesota Department of Health’s (MDH) Children and Youth with Special Health Needs (CYSHN) Section. This funding opportunity seeks to enhance the health care of birthing parents in Minnesota through the implementation of the IMPLICIT model in clinical practice. The overarching goal is to prevent birth defects by addressing modifiable risk factors such as smoking, maternal depression, multivitamin/folic acid use, and family planning. The IMPLICIT model integrates interconception care into well-child visits and has been shown to reduce risks associated with preterm and low birth weight infants. Eligible applicants include individuals, not-for-profit organizations, for-profit organizations, and health centers with clinical quality improvement experience. Individual clinics and local jurisdictions are not eligible to apply directly but can participate as implementation sites recruited by the awarded applicant. Collaboration between multiple entities is not permitted for this grant. Applications must be submitted via the Foundant online portal by 4:30 p.m. Central Time on July 11, 2025. A non-binding Notice of Intent is strongly encouraged and should be submitted by June 20, 2025. An informational Microsoft Teams meeting will be held on June 18, 2025. The grant provides a total of $130,000 annually to fund one award over a three-year period, with funding beginning on September 1, 2025, and ending on June 30, 2028. Clinics will receive an annual stipend of $18,000 to support the implementation of the IMPLICIT model and cover the required membership to the national IMPLICIT Network. There are no matching fund requirements for this grant. The grantee will be responsible for recruiting at least five clinics to implement the IMPLICIT model, providing technical assistance, supporting data collection and reporting, ensuring sustainability, and facilitating collaboration with the IMPLICIT Network and MDH. Applications must include several required documents: a Grant Applicant Face Sheet, Project Narrative, Work Plan, Budget Details and Justification, Conflict of Interest Form, Due Diligence Form, and Indirect Cost Questionnaire. The Project Narrative should detail the applicant’s organizational capacity, expertise of key staff, recruitment strategies for clinics, plan for facilitating training and support, strategies for overcoming potential barriers, and sustainability planning. The Budget section must provide detailed calculations and justifications for salary and fringe benefits, contractual services, travel, supplies and expenses, and indirect costs. Applications will be evaluated on a 100-point scale by a review committee. Scoring criteria include the applicant’s organizational capacity, skills and knowledge, linkages and collaborations, project description, work plan, and budget. Progress reports are required quarterly, with a final report due by July 31, 2028. MDH will conduct at least one monitoring visit and financial reconciliation during the grant period.
