Funded projects must incorporate evidence-based interventions aligned with service strategies and key performance indicators set by DC Health, including measures of postpartum visit attendance, breastfeeding duration and exclusivity, mental health service access, and the development of medical homes for CYSHCN. The total anticipated funding amount for this program is $1,750,000, with up to seven awards to be distributed. Individual award amounts will range between $50,000 and $250,000 annually. The initial period of performance runs from October 1, 2025, through September 30, 2026, with the possibility of four additional 12-month continuation periods based on performance and funding availability. There is no matching or cost-sharing requirement for this program. All expenditures must be allowable, allocable, and justified in accordance with the approved budget and work plan. Eligible applicants include nonprofit organizations, primary care clinics, Federally Qualified Health Centers (FQHCs), and for-profit clinics providing primary care services. Individuals are not eligible to apply. All applicants must be registered in the federal System for Award Management (SAM) and in DC Health’s Enterprise Grants Management System (EGMS). A Tax ID and active Unique Entity Identifier (UEI) are required. Required application materials include business licensure, proof of insurance including cyber liability coverage, IRS documentation (for nonprofits), project narrative, work plan, budget details, risk assessment, letters of support, and more. Proposals that are incomplete or fail to meet eligibility requirements will not be reviewed. No letter of intent is required, but the final date to submit questions for the FAQ is July 22, 2025.
