Infection Prevention Liaison Program

The Illinois Department of Public Health (IDPH) is offering a funding opportunity under the Infection Prevention Liaison Program (IPLP), supported through federal pass-through funds from the CDC’s Epidemiology and Laboratory Capacity for Prevention and Control of Emerging Infectious Diseases (ELC) Program. This grant program is managed by the IDPH Office of Performance Management and aligns with the department’s broader strategy to promote health equity and support effective infection prevention measures across healthcare facilities in Illinois. The primary goal of the grant is to enhance healthcare provider knowledge and implementation of infection prevention and control practices in long-term care facilities, selected acute care facilities, and local health departments. The selected grantee will work closely with the IDPH’s Healthcare Associated Infections and Antimicrobial Resistance Prevention Program and the Regional Infection Prevention Program to deliver technical assistance, conduct assessments, and provide training. Key responsibilities include conducting infection prevention education, delivering webinars, performing on-site and remote assessments using CDC tools, and engaging leadership within nursing homes to promote infection control best practices. Applicants are required to submit their application through the IDPH’s electronic grants system, EGrAMS. All applicants must be registered and qualified through the Illinois Grant Accountability and Transparency Act (GATA) portal before submission. Required application components include a project narrative, budget and budget narrative, and two letters of support. Only entities that are not public housing organizations or foreign applicants are eligible. Each applicant must also have an active UEI number and registration in SAM.gov. Applications will be evaluated based on several criteria, including demonstration of need, organizational capacity, alignment with project requirements, detailed work plans, SMART objectives, and justifiable budgets. A significant portion of the scoring will emphasize health equity integration using IDPH’s Health Equity Checklist. The checklist evaluates applicants' plans for addressing disparities and integrating culturally appropriate services and social determinants of health. A mandatory technical assistance session will be held on June 5 and June 17, 2025, and all prospective applicants must attend one of these sessions. The application window opens on June 2, 2025, and closes on July 1, 2025, at 5:00 PM CST. The anticipated award notification date is June 2, 2025, with the project period beginning on September 1, 2025, and ending on August 31, 2026.
