Grant Description

The New York State Department of Health, Division of Chronic Disease Prevention, through its Tobacco Control Program (TCP), is offering funding under Component B: Center for Health Systems Improvement. This program seeks to strengthen and enhance the capacity of health care systems across New York State to deliver care that aligns with evidence-based clinical guidelines for the treatment of tobacco use and nicotine dependence. The initiative focuses on systems and policy-level changes within healthcare organizations to improve the consistency and quality of tobacco cessation care provided to patients. The Bureau of Tobacco Control (BTC), which administers the TCP, has a mission to reduce illness, disability, and death associated with tobacco use and secondhand smoke exposure. Its evidence-based approach integrates policy advocacy, system-level intervention, community engagement, and media campaigns to shift social norms around tobacco and nicotine use. Through this funding opportunity, the Department seeks organizations that can engage healthcare systems to embed tobacco use disorder screening, treatment, and referral mechanisms into routine care processes. The supported projects will be expected to utilize current cessation science, leverage data to inform improvement, and support organizational policy change to ensure the sustainability of tobacco cessation interventions. Funds may be used for activities such as workforce training, system integration, patient referral pathways, and evaluation of system-level outcomes. The application process includes a strongly encouraged but optional Letter of Interest (LOI), which must be submitted by October 23, 2025. Questions regarding the RFA are due the same day, and the Department will post responses by November 12, 2025. Applications must be submitted through the Statewide Financial System by December 5, 2025, at 4:00 PM Eastern Time. Awards are anticipated to be announced by February 1, 2026, with contracts expected to span 60 months from the award date. Eligible applicants include governmental entities and not-for-profit organizations operating in the field of health services within New York State.
