The U.S. Department of Health and Human Services (HHS), through the Administration for Community Living (ACL) and its Administration on Aging (AoA) division, is offering a cooperative agreement opportunity under the title "Expanding Strategies for Community Evidence-Based Falls Prevention Programming." The cooperative agreement will provide two awards of $1,250,000 each for a fully funded 36-month project and budget period, with a project start date of September 1, 2025. The opportunity is open to domestic public or private nonprofit entities including state and local governments, federally recognized and non-recognized tribal entities, institutions of higher education, school districts, public housing authorities, and community and faith-based organizations. Matching funds are not required for this award. However, if an applicant proposes voluntary non-federal contributions, they will be held accountable for them if accepted in the project budget. Applications must be submitted electronically through Grants.gov by 11:59 p.m. Eastern Time on August 15, 2025. Applicants are encouraged—but not required—to submit a letter of intent by the same date to aid ACL in the review planning process. All required documents, including the project narrative, budget justification for all three years, and letters of commitment from key partners and program trainers, must be submitted with the application. Applications that fail to meet the deadline or submission criteria will not be reviewed. Evaluation criteria include the clarity and alignment of the project’s goals with the NOFO objectives, strength and specificity of partnerships, demonstrated capacity for person-centered program delivery, sustainability planning, and a comprehensive evaluation strategy. The project narrative must detail planned outreach strategies, geographic reach, measurable outcomes, and mechanisms to collect and report de-identified participant data. Participation in annual professional development and regular reporting to the National Falls Prevention Resource Center is required.
