Grant Description

The New York State Office of Mental Health (OMH) is seeking proposals for its Expansion of Peer Bridger Services initiative, intended to facilitate successful transitions from inpatient psychiatric care to community living. This grant targets Article 28 hospitals that operate inpatient psychiatric units and are committed to integrating the Peer Bridger model—an evidence-based, person-first approach grounded in peer support and mutual aid. Through this program, each awarded hospital will receive $300,000 annually for five years, totaling $1.5 million per site and $4.5 million across three facilities statewide. Peer Bridger services are distinguished by their emphasis on voluntary, peer-led support that begins during hospitalization and extends into the community post-discharge. The model empowers participants by fostering hope, independence, and life engagement through mentorship, systems navigation, and individualized skills training. These services are not a substitute for care management but complement it by reinforcing self-determination and self-efficacy through shared lived experiences. Bridgers also help clients develop Wellness Recovery Action Plans (WRAP), psychiatric advanced directives, and other personal tools as they navigate the reintegration process. Eligible applicants must be Article 28 facilities with psychiatric inpatient units. Only three awards will be granted, one per OMH region (New York City, Long Island, Hudson River, Central NY, and Western NY). The RFP outlines a robust application process, requiring applicants to prequalify through the New York Statewide Financial System (SFS) and submit proposals via SFS by January 15, 2026. Additional steps include notifying Local Governmental Units (LGUs) of the intent to apply and adhering to all formatting requirements. No mailed or emailed applications will be accepted. Applications will be evaluated based on a 100-point system, assessing factors such as program design, implementation plans, performance monitoring, and diversity, equity, and inclusion strategies. Applicants must also submit a detailed operating budget using Appendix B, ensuring administrative overhead does not exceed 15%. The evaluation also considers the organization's ability to retain certified peer specialists and establish trauma-informed, culturally competent care practices. The anticipated timeline includes an RFP release on November 6, 2025, a Q&A period ending December 1, 2025, and award announcements expected by February 18, 2026. Contracts will commence on July 1, 2026, and extend for five years. The program is non-recurring and contingent on continued state appropriations. Pursuant to the New York State Division of Budget Bulletin H-1032, dated June 7, 2013, New York State has instituted key reform initiatives to the grant contract process which require not-for-profits to be Prequalified in order for proposals to be evaluated and any resulting contracts executed. Proposals received from eligible not-for-profit applicants who have not been Prequalified by the proposal due date of 2:00 PM EST on the Proposal Due Date posted in section 2.3 will not be able to submit their bid response through SFS.
