The Community Health Improvement Grants program, offered by CommonSpirit Health, is designed to enhance the health and well-being of vulnerable and underserved populations within the communities served by their hospitals. This program supports initiatives that address significant community health needs as identified in each hospital's most recent Community Health Needs Assessment (CHNA) or implementation strategy. CommonSpirit Health operates a network of hospitals nationwide, but the Community Health Improvement Grants program is only available through its fully-owned hospitals, excluding those located in Utah, Colorado, and Kansas, which have separate funding programs. Grant applicants must be either a registered IRS 501(c)(3) nonprofit or have a qualified fiscal agent with the same designation. Eligible proposals must focus on projects serving the hospital’s service area and target underserved or vulnerable populations, which may include members of the broader community, CommonSpirit patients, or both. Projects must run for a period between nine and twelve months and may not include unrestricted general operating support, short-term events under nine months, capital construction, renovations, lobbying, or political activities. Funding for equipment, including computer hardware, is limited. The program offers two primary funding categories: small grants of up to $19,500 and standard grants ranging from $20,000 to $150,000. Projects requesting $20,000 or more must involve at least one external partner organization, aside from the hospital, that plays a distinct and substantive role in the project. All grants are for one-year periods only, although organizations may apply in subsequent years for related or new projects, up to a maximum of four consecutive years of funding. Applications are accepted online during the annual application window, which for the 2025 cycle runs from July 14, 2025, at 9:00 a.m. PDT to September 12, 2025, at 11:55 p.m. PDT. Funding decisions are announced on or about January 19, 2026, with grant agreements finalized in February and March 2026. The grant period begins April 1, 2026, with mid-year project reports due November 13, 2026, and final reports due May 16, 2027. Technical assistance and guidance are available from local grant leaders or by contacting the central program office. Proposals will be assessed based on their alignment with identified community health needs, the degree to which they strengthen service delivery systems, and the potential for sustained positive impact.
