Pathways to Health Grant

Riverside Community Health Foundation (RCHF), a nonprofit public benefit organization based in California, is currently accepting applications for its Pathways to Health 2026 Grant Opportunity. This funding initiative supports organizations that aim to address critical health disparities in the Greater Riverside Area through sustainable, equity-driven programming. RCHF's mission is to improve the health and well-being of communities it serves, and its vision is to cultivate a healthier and more active population for future generations. Through this grant, RCHF will fund targeted efforts that align with its ongoing commitment to address the complex social determinants of health in underserved areas. Eligible applicants may request funding of up to $40,000 per project. The foundation may also exceed this cap if it deems an application especially compelling. Funded programs must operate within one of three focus areas: Access to Healthcare, Mental Health and Behavioral Wellness, or Chronic Disease and Preventive Care. Each applicant must select one primary focus area when submitting a proposal. Example initiatives may include expanding mobile health services, enhancing mental health support networks, or delivering community-based education to promote chronic disease prevention and management. Applicants must be 501(c)(3) nonprofit organizations, government entities, or Native American Tribal governments or organizations. Additionally, eligible projects must directly serve residents within the city limits of Jurupa Valley, Riverside, Moreno Valley, Corona, Norco, and Perris. Only the following ZIP codes are eligible under this program: 92501, 92502, 92503, 92504, 92505, 92506, 92507, 92508, 92509, 92518, 92521, 92522, 92551, 92553, 92557, 92570, 92571, 91752, 92860, 92879, and 92881. Organizations must serve the public equitably, without discrimination based on gender, sexual orientation, ethnicity, religion, age, disability, or other protected characteristics under California Civil Code Section 51 and U.S. Code Title 42, Section 18116. Applications are due by Monday, October 6, 2025, at 5:00 PM. All applications must be submitted through the RCHF online grants portal. Organizations that have previously applied should use their existing user accounts. New applicants must create a user profile and complete an eligibility quiz before gaining access to the full application. Required attachments include a tax exemption letter, list of officers and directors, organizational budget, IRS Form 990, an audited financial statement, and a signed Non-Discrimination Affirmation. Additionally, applicants must submit a project-specific budget using RCHF’s provided template. All proposals will be reviewed by the RCHF Grants Team. Evaluation criteria will focus on alignment with funding priorities, geographic eligibility, program feasibility within the proposed budget, clearly defined outcomes and metrics, and the applicant’s organizational capacity to execute the project. Award announcements are expected within the first two weeks of December 2025. The grant period will span from January 1, 2026, through December 31, 2026.
