Any unspent funds at the conclusion of the award period must be returned to the Arizona Department of Administration. Eligibility for this grant is highly restrictive. Applicants must operate pursuant to Public Law 93-638, function as a critical access hospital under the definitions set forth in federal regulations, and be located on unincorporated or tribal land in a county in Arizona with a population not exceeding 70,000 people. Facilities must demonstrate that they benefit tribal members in need of dialysis treatment, serve a patient population of at least 10,000 people, and be located at least 30 miles from the nearest outpatient dialysis treatment center licensed under Arizona statutes. These criteria ensure that the facility funded will address underserved populations without duplicating existing services. Applications must be submitted through the eCivis system. Applicants are required to provide detailed responses including a scope of work, a project timeline, and an evaluation plan. The scope of work should summarize the need, objectives, and performance measures while demonstrating compliance with eligibility conditions. The project timeline must outline key objectives, activities, and milestones within the twelve-month period. The evaluation plan should describe how program success will be measured, who will provide oversight, and how issues will be addressed. In addition, applicants must provide a budget summary and narrative explaining the basis for each requested expense category and supporting documentation such as contractor quotes where applicable. Evaluation of applications will be based on four scored criteria. The scope of work will be reviewed for the quality of need assessment, objectives, and performance measures. The project timeline will be judged by the level of detail and feasibility for meeting grant requirements. The evaluation plan will be assessed on oversight capacity and issue resolution methods. Finally, budget alignment will be scored based on the clarity and justification of expenses and their alignment with the overall project. Only applications that present clear, convincing documentation of capability and alignment with program requirements will be considered. The grant program will follow a strict timeline. Applications are due by September 25, 2025, and awards will be announced by September 30, 2025. The funding period will begin on October 1, 2025, and end on September 30, 2026.
