Grant Description

Full Term First Birthday (FTFB) is a collective impact initiative coordinated by Summit County Public Health (SCPH) in Ohio. The program is dedicated to improving maternal and infant health outcomes in Summit County, with a focus on reducing the infant mortality rate, which stood at 7.1 per 1,000 live births in 2022. In response to statewide health priorities and in partnership with the Ohio Department of Children and Youth (DCY), SCPH has opened a funding opportunity for nonprofit and faith-based organizations to support community-level projects aimed at improving outcomes for pregnant women and families with infants up to one year old. Eligible applicants include community- and faith-based organizations located in Summit County. Selected proposals will be incorporated into SCPH’s broader grant application to DCY. If SCPH’s proposal is awarded by DCY, sub-grantees will receive funding to implement the services proposed. The grant period spans January 1, 2026 through June 30, 2027, and individual applicants may request up to $150,000. This is a reimbursement-based grant, requiring sub-recipients to have the financial capacity to incur and cover eligible expenses prior to reimbursement. Funded programs must address at least one of five key social or physical determinants of health: access to early prenatal healthcare, access to quality education, community connectedness, financial stability, and environmental quality including housing and neighborhoods. Services that may be funded include physical support (such as supplies, materials, incentives, crisis assistance, and direct services) and social support activities (such as baby showers, parent education, community organizing, or leadership development). Some costs are not allowed, including capital improvements, equipment purchases, Medicaid-reimbursable services, and items already funded through state programs. The grant application was released on September 3, 2025. A technical assistance session will be held on September 8, 2025, at 10:00am via Google Meet. The application deadline is September 17, 2025, at 4:00pm, and selected applicants will be notified no later than September 30, 2025. SCPH will submit the compiled grant application to DCY by October 2, 2025. Applicants must submit a completed proposal form, detailed program budget with justifications, and Form 7c for subgrantee/subcontractor utilization. Proposals will be evaluated on alignment with program goals, the ability to track demographic and service delivery data, and measurable success outcomes, such as the number of families served and expected program impact. This is not a direct application to DCY, but rather participation in a coordinated, community-wide application led by SCPH.
