Grant Description

The My Healthy Baby Infrastructure Grants 4.0 is a funding opportunity administered by the Indiana Department of Health (IDOH), specifically through its Maternal and Child Health Division. This grant aims to enhance the capacity of organizations delivering perinatal home visiting services by supporting one-time infrastructure costs. The initiative stems from funding authorized under House Enrolled Act 1007 (IC 16-35-1-11 Sec.11), with an overarching goal to ensure coordinated and high-quality voluntary home visiting services for expectant and new families across Indiana. Eligible applicants include organizations that are part of the My Healthy Baby referral network and have signed partner agreements. However, consideration is also extended to external programs if they can demonstrate a solid foundation and use of evidence-based or Indiana-specific perinatal home visiting models. These models must primarily use voluntary perinatal home visits as their core service, must be established in the local community for at least one year, and should target key maternal and child health outcomes such as breastfeeding, safe sleep, tobacco cessation, and birth spacing. Furthermore, they must serve clients from pregnancy through at least the first year post-birth. Award funding is intended exclusively for infrastructure-related expenditures and does not cover staffing, mileage for visitations, indirect or other human resources costs. Examples of acceptable uses include professional development related to My Healthy Baby standards (e.g., lactation counselor training, participation in conferences), mental health training, technology upgrades to improve service delivery, language access enhancements, and expenses tied to client enrollment and retention. Applicants can request funding across one or more of these categories and must justify the use and benefit of each expenditure in the provided template. The total anticipated funding available is $200,000, with the exact number and size of awards to be determined based on need and application quality. Applications do not require cost-sharing or matching funds. The project period spans from September 1, 2025, to June 30, 2026. Interested applicants must submit their completed applications by Friday, July 11, 2025, at 5:00 p.m. EST to the email address myhealthybaby@health.in.gov. Submissions must use the provided application template without modifications. Font readability and completeness are emphasized, and late or improperly formatted applications will not be considered. Important timeline dates include the RFA release on June 10, 2025, a webinar for applicants on June 24, 2025, and the deadline for submitting written questions by June 30, 2025. Responses to questions will be posted weekly starting June 19, 2025, with final responses available by July 3, 2025. Award announcements are expected on August 1, 2025. Interim reports are due February 28, 2026, and final reports must be submitted by July 31, 2026. Applications will be reviewed based on completeness and demonstrated need. For inquiries or assistance, applicants can contact Iris Palacios, the My Healthy Baby Coordinator, at 317-447-0677 or via email at MyHealthyBaby@health.in.gov. The opportunity does not currently indicate a recurring or rolling application model, but the timeline structure suggests an annual cycle may be anticipated.
