Grant Description

The grant opportunity titled “Expanding Strategies for Community Evidence-Based Falls Prevention Programming” is offered by the Department of Health and Human Services (HHS) through the Administration for Community Living (ACL). This forecasted funding initiative seeks to build upon ongoing efforts to reduce fall risks and incidents among older adults and adults with disabilities. The ACL focuses on enhancing the quality of life for older adults and people with disabilities through various community-based initiatives, including health-related programs such as this one. The primary purpose of this grant is to support cooperative agreements that will explore and implement targeted partnerships and innovative strategies to expand evidence-based falls prevention programs. This includes strengthening collaborations, developing and disseminating educational resources, and identifying and sharing best practices. Successful applicants will also design and implement evaluation methods to assess the impact of these strategies on reducing fall risks. This funding opportunity anticipates awarding two cooperative agreements, with each award set at $1,250,000, and a total funding pool of $2,500,000. Projects funded under this opportunity must begin on September 1, 2025, which is also the estimated award date. The grant falls under the Assistance Listing 93.761 and does not require cost sharing or matching funds from applicants. Eligible applicants include a wide range of entities such as state and local governments, Native American tribal organizations (both federally and non-federally recognized), independent school districts, public and private higher education institutions, special district governments, public housing authorities, and 501(c)(3) nonprofits. Notably, foreign entities are excluded from eligibility. Faith-based and community organizations that meet eligibility requirements are encouraged to apply. Applications are expected to open on April 23, 2025, and will close on June 23, 2025. The opportunity is not offered on a rolling basis, but it is expected to recur in future cycles. There are no preliminary deadlines such as Letters of Intent or RFIs required before the final due date.
