Grant Description

Highmark Blue Cross Blue Shield Delaware launched the BluePrints for the Community grant initiative in 2007 to support health-focused projects that address the needs of Delaware's uninsured and underserved populations. Administered through a donor-advised fund at the Delaware Community Foundation, the program has committed over $28 million through more than 300 grants as of 2022. The grant supports proposals that aim to improve access to healthcare or reduce health disparities within Delaware. Eligible projects must align with one or more of five strategic focus areas: Healthcare Access, Economic Stability, Social and Community Context, Neighborhood and Built Environment, and Health Workforce. Examples include initiatives promoting preventative care, addressing housing or food insecurity, supporting mental health services, improving environmental health, or enhancing provider education and training. Eligible applicants include incorporated nonprofit 501(c)(3) organizations and community-benefit government agencies such as libraries, park systems, or public school districts. Individuals are not eligible. Prior grantees and those with open applications may submit proposals for new initiatives. Conflict-of-interest disclosures are required for organizations represented on relevant boards or councils. The Large Grant cycle is designated for funding requests above $50,000. Applications exceeding $200,000 require a preliminary meeting with BluePrints representatives. Large Grant applications are accepted annually during the window from April 1 to May 31, and should be submitted via the Delaware Community Foundation's online portal.
