HRSN Community Capacity Building Funds 2025
This funding opportunity supports community organizations in Oregon to enhance their capacity to provide essential health-related social services, such as housing and nutrition support, to vulnerable populations enrolled in the Oregon Health Plan.
The 2025 Health-Related Social Needs (HRSN) Community Capacity Building Funding (CCBF) grant supports the development and expansion of organizations capable of providing HRSN benefits to qualifying Oregon Health Plan (OHP) members. HRSN benefits include climate supports, housing benefits, nutrition supports, and outreach and engagement services. This initiative addresses the understanding that health outcomes are deeply affected by social conditions such as housing, food security, and access to environmental resources. The CCBF initiative is part of a broader strategy by Oregon’s Medicaid program to integrate social needs into healthcare delivery, particularly for populations undergoing specific life transitions, including individuals recently released from incarceration, those discharged from behavioral health facilities, youth involved in child welfare, individuals transitioning to dual Medicaid/Medicare eligibility, the homeless or at-risk populations, and young adults with special healthcare needs. Over $55 million is available statewide for 2025, managed by Oregon’s Coordinated Care Organizations (CCOs). Eligible applicants include community-based organizations, local governments, housing agencies, food and nutrition service providers, case management providers, traditional health worker organizations, and other entities supporting HRSN benefit delivery. Applicants must demonstrate financial stability and a commitment to delivering culturally and linguistically responsive and trauma-informed services. Applications must be fully completed, including a signed attestation and a detailed budget request, by May 30, 2025. Applications require organizations to detail the counties they will serve, populations targeted, languages offered, cultural responsiveness, and trauma-informed practices. Applicants must also describe strategies for delivering specific HRSN benefits and how requested funds will build operational capacity. Eligible funding uses include technology acquisition, development of business practices, workforce development, and outreach and education activities. Ineligible uses include real estate, vehicles, ongoing operating costs, and non-HRSN related activities. Awards will be evaluated based on the applicant’s ability to justify their funding request, the strength of their service delivery plans, their history or plan for providing culturally responsive care, and their readiness to promote health equity. Recipients must submit annual reports detailing use of funds, activities completed, and progress toward HRSN benefit delivery goals. Each CCO may have slightly different application submission processes, so applicants must check individual CCO guidelines.
