The Agency Health Priority Grant Program is offered by the Intermountain Community Care Foundation (ICCF), a private philanthropic entity dedicated to supporting Intermountain Health’s identified health priorities. The Foundation’s mission is to improve access to healthcare services and increase healthy behaviors for low-income, uninsured, or medically underrepresented populations in Utah and Southeast Idaho, specifically Cassia County and Minidoka County. Through targeted funding, ICCF seeks to strengthen programs and services that address the region’s most significant health needs, as identified through Community Health Needs Assessments (CHNA). The Agency Health Priority grant focuses on three main areas: mental well-being, chronic and avoidable health outcomes, and access to care. Eligible mental health projects may aim to reduce suicide deaths, address frequent mental distress such as anxiety and depression, and decrease opioid misuse. Chronic illness prevention efforts can target reducing high blood pressure, preventing diabetes, and increasing immunization rates. Access initiatives should improve the availability of comprehensive medical, dental, and behavioral healthcare services, particularly for underserved populations. Funding requests must range between $5,000 and $50,000 for the entire project period, with the stipulation that the request cannot exceed 20 percent of the applicant organization’s annual operating budget. Small equipment and materials are eligible for support up to $5,000, but large equipment, construction, and capital costs are ineligible. Consideration may be given for computer equipment if it is vital to the project’s success. Awarded organizations will be expected to submit outcome reports and track aggregated project and participant data to demonstrate measurable human impact. Eligible applicants must be 501(c)(3) nonprofit organizations and must submit a complete financial audit conducted within the two years preceding the application. Applications also require a W-9 form, a completed supplier form, and supporting organizational documents including a current list of the board of directors, key staff, and itemized budgets for both the organization and the proposed program. All applications are submitted through ICCF’s online portal, hosted by Versaic, which requires account creation before submission. The program operates two competitive funding cycles annually. To be considered at the May Board meeting, the application window is January 1 through February 1. For the November meeting, applications are accepted from July 1 through August 1. Grants are not awarded for large capital projects, political activities, religious programs that do not meet nondiscrimination criteria, or projects that duplicate services with existing reimbursement programs, among other exclusions. Organizations funded by ICCF may not reapply for 18 months after being funded, and only one active grant may be held at a time. Applications are reviewed by ICCF staff for alignment with program criteria before being evaluated by a designated committee. This committee’s recommendations are submitted to the Foundation Board for final decision-making. Approved grants require a signed agreement, after which payments are generally made in full. The Board or staff may conduct site visits or program reviews at any time to ensure compliance with grant terms.
