Applications are submitted in three separate PDFs: the application, financials, and the latest Form 990. All documents must be submitted via email to the same address as the LOI, following precise formatting instructions. Deadlines for applications are 30 days from invitation or 30 days prior to the board meeting—whichever comes first—with submissions due by noon Arizona time on the final day. Post-award requirements include a formal acknowledgment letter within 30 days of receiving the check, and a completion letter within one year or upon project completion, whichever comes first. This report must confirm that funds were used for the approved purpose and describe the outcomes. The Foundation is strict about adherence to guidelines, and any deviation—including conflicts of interest, misrepresented use of funds, or multiple submissions from divisions under the same EIN—can result in immediate disqualification.
