This grant provides financial support to nonprofit organizations in Allegheny County, Pennsylvania, to develop community programs that improve health and well-being by addressing social determinants and reducing health disparities.
The ACMS Foundation Grant supports initiatives that aim to foster healthy communities and family environments within Allegheny County, Pennsylvania. As the philanthropic arm of the Allegheny County Medical Society, the foundation is dedicated to improving wellness by addressing social determinants and reducing health disparities. The foundation prioritizes organizational-based activities that actively involve individuals and families in the planning, development, and implementation of community-level programs, thereby strengthening local capacities to build healthier environments. The 2025–2026 ACMS Foundation Grant cycle is now open, with applications accepted from August 1, 2025, until September 15, 2025. Grants are awarded to nonprofit organizations that directly serve communities within Allegheny County. Funded projects must either address pressing community needs or present innovative approaches to ongoing societal challenges. Furthermore, the foundation favors proposals with potential for broader impact or replication, particularly those aligned with its mission of advancing wellness through confronting social determinants of health. Eligible applicants are strictly limited to nonprofit organizations operating in Allegheny County, Pennsylvania. Proposals must demonstrate how the initiative will affect the local community and promote individual and collective well-being. Applications are submitted through the ACMS Foundation’s online portal. To apply, organizations must first create an account on the portal, complete all required fields in the application, upload all requested attachments (including a completed budget template), and submit the application through the system. The review process involves a set of clearly defined criteria. Reviewers evaluate whether a project is responsive to local health-related needs, the innovativeness or urgency of the proposal, its potential for replication, and the extent to which it furthers the foundation’s mission. Funding decisions will be made by October 28, 2025, and funds are expected to be disbursed by November 30, 2025. The application is not described as having pre-application steps such as Letters of Intent or Concept Papers. Technical support related to the grant portal is available from Melanie Mayer, while eligibility and application content questions should be directed to Sara Hussey, the Executive Director of the ACMS Foundation.
